A photo of your government issued ID and work ID on a piece of paper with the date of your requested booking written on it.
Two references from established independent providers you’ve seen in the past 6 months.
If none of the above are possible, please provide my assistant with your full legal name, employer name, and title. She will advise as to what screening information we will accept.
